Robotic Fellowship Grant Instructions
Institutions invited to apply for the Fellowship grant will complete the attached full application. Enclose a
Brief Biographic Summary (not a complete curriculum vitae) for the fellowship director as well as any other surgeons with whom the fellow will work. The application should include goals and objectives for the fellowship, fellowship curriculum, evidence of adequate clinical training, evaluation methods, evidence of adequate supervision and training, and opportunities for clinical research. Please forward these with the application.
Deadline November 1, 2010
- Complete the application cover sheet and submit with the application. The body of the application should be single-spaced. Please use a common 12 or 14 point type font.
- Indirect costs are not paid to the recipient or to the recipient’s institution.
- The grant must be used in the 2010-2011 academic year. It cannot be postponed.
- Structure: The application must contain the following sections in the following order.
- Letter from current chair supporting the grant application
- Goals and Objectives
- Curriculum
- Evaluation methods
- A list of surgeons with whom fellow will work
- Robotic clinical volume at institution for the past 12 months
- Total number of surgical procedures you anticipate the fellow will perform during the fellowship
- Planned Research opportunities
- Percent of time fellow will spend in surgery, with patients, on research activities, teaching and performing administrative duties
- Submit a 2 page Brief Biographical Summary for the fellowship director. Do not include a complete curriculum vitae
- Submit a 2 page Brief Biographical Summary for each faculty with whom the fellow will work

- Deadline: 6. Deadline: Invited grant proposals must be received by November 1, 2010. The applications will be reviewed and a decision for funding will be made by December 1, 2010.

Policies and Agreements
If funded, the successful Robotics Fellowship Program must provide:
- Name and contact information for the selected Robotics Fellow
- Protected time for the Robotics Fellow to attend training sessions at ISI
- List of all research activities accomplished by the Robotics Fellow during training including:
- Publications submitted, accepted or published
- Presentations
- Abstracts
- Book Chapters
- Posters
- Other research activity
- Number of robotic cases in which the Robotics Fellow participated including summary of types of cases
- Amount of time the Robotics Fellow spent at the patient side and at the console
- Overall evaluation of the Robotics Fellowship by the Robotics Fellow and the fellowship director
- Accounting of how the Robotics Fellowship funds were utilized
